A 65-year-old Thai male patient presented with significant weight loss and progressive renal dysfunction for 1 month. He was recently diagnosed allergic rhinitis multiple mononeuropathy. had elevated serum creatinine of 5 mg/dl (Baseline 1.2 mg/dl), high anti-myeloperoxidase titer IgG4 level. biopsy revealed extensive cellular fibrocellular crescents severe tubulointerstitial nephritis IgG4-positive plasma cell granuloma, suggesting a diagnosis overlap p-ANCA associated glomerulonephritis IgG4-related kidney disease.
